Advanced Primary Care ECHO!
ConferMED, in partnership with the Weitzman Institute, is pleased to announce a new opportunity for ConferMED providers: Advanced Primary Care ECHO. Advanced Primary Care ECHO connects primary care medical and behavioral health providers across the country with a multidisciplinary team of experts, using twice-monthly videoconferencing sessions to explore key topics to improve quality of care and clinical outcomes for underserved patients.
During each session faculty present a brief didactic on relevant topics and then provide actionable recommendations to challenging cases presented by participants.

Continuing Education Credits Available
CME, CNE, CPE, and CEU credits will be provided for all sessions for physicians, nurse practitioners, physician assistants, nurses, and clinical social workers, among others, by Community Health Center, Inc., an accredited provider through Joint Accreditation for Interprofessional Continuing Education.
This ECHO is being offered as part of the HRSA Telehealth Technology-Enabled Learning Program (TTELP) in order to improve rural health outcomes.
