ConferMED, PC and ConferMED of California, PC (collectively, “ConferMED”) value your partnership and recognize the critical role you play in helping us deliver important healthcare services. A key component to ConferMED’s work is ensuring our specialty network’s compliance with all applicable federal and state requirements. As a member of ConferMED’s specialty network we are asking you to certify that you are committed to ensuring compliance with all applicable federal and state requirements.
Identified below are three components of ConferMED’s compliance program: Compliance Education, Fraud, Waste, and Abuse & General Compliance Training and HIPAA Privacy and Security Training. For each component, you will need to determine if you have completed it with your organization or an organization you are affiliated with, and if not by accessing the training ConferMED has provided below.
At the bottom of this page there is an attestation button that will take to you to a form where you can select the appropriate method of training completion that applies to you. By completing the following attestation, you certify that you are committed to ensuring compliance with all applicable federal and state requirements. Please complete the form in its entirety.
Here is what you can expect on the form:
ConferMED’s Compliance Education
ConferMED is committed to compliance and maintains a compliance program, which outlines its compliance expectations of staff and vendors, reporting mechanisms and other important compliance matters (“Compliance Program”). By signing this attestation, you acknowledge that you have accessed and read the Compliance Program document (link below), you understand your or your organization’s compliance obligations and you have shared the Compliance Program document with those engaged in work for ConferMED. The Compliance Program document can be accessed here: https://confermed.com/compliance-policy/
- I reviewed the Compliance Program document and I agree to comply with the expectations including the Code of Conduct. I will review it annually.
OR
- My organization or an organization with which I am affiliated utilizes its own Code of Conduct, which is comparable to ConferMED’s Code of Conduct and which I review annually.
Fraud, Waste, and Abuse & General Compliance Training:
Employees and independent contractors of ConferMED are required to complete General Compliance and Fraud, Waste and Abuse training upon engagement with us and annually thereafter.
- I completed the CMS Fraud, Waste and Abuse training at https://www.cms.gov/Outreach-and-Education/MLN/WBT/MedicareFraudandAbuse/FraudandAbuse/story.html, and will complete this or a similar training on annual basis thereafter and maintain documentation of the same.
OR
- My organization or an organization with which I am affiliated trains all personnel upon hire or engagement and on an annual basis thereafter on Fraud, Waste, and Abuse and/or General Compliance and maintains documentation of the same.
HIPAA Privacy & Security
In accordance with regulatory requirements under the HIPAA Privacy and Security Rules, ConferMED must demonstrate that it has implemented a training program for all members of its workforce on the policies and procedures with respect to the privacy and security of protected health information.
- I completed the HIPAA Privacy and Security Training offered by ConferMED at: HIPAA Privacy and Security, and will complete this or a similar training on annual basis thereafter and maintain documentation of the same.

- My organization or an organization with which I am affiliated trains all personnel upon hire or engagement and on an annual basis thereafter on HIPAA Privacy and Security and maintains documentation of the same.
